Medication Policy
Some students may require medication during school hours. Please follow these procedures:

1. Parents should make the school aware of any medical problem a child might have. School employees may not administer medicine to any student unless the school has the appropriate form signed by the child's doctor.

2. An Authorization for Medication form must be filled out by the physician and signed by the parent. This form is available in the school office.

3. Medications should be brought in the original container to the office by a parent with the Authorization for Medication form. Parents must initial the Receipt of Medication form in the office.

4. Children should not bring any medication to school.

5.  Students can not carry any medication for self-administration unless approved by the school nurse after proper paperwork has been submitted.  The only medication that a student may carry (by state law) is an asthma inhaler, an epinephrine injector, or diabetes medication and supplies.  These all must have proper paperwork submitted before student can carry them.

You can obtain one of these forms from the front office or download it from the link on this page. You also need to know that the medication will usually not be given by a nurse. Because the nurses are only able to be at the school one day a week, other staff (usually the secretaries) will give the medication (once we have the signed form).

We want your student to have access to the medication if they need it, so please get this form to the school as soon as possible. If the school doesn't have the form and medication is needed they will call parents to come administer it, or will call 911 if no parent is available and emergency care is needed.

By clicking the "Medication" link below you will find the district required Medication Authorization form, along with those State required forms for asthma inhaler, epinephrine injectors, and Glucagon.  By law these forms are required to be completed by physician and parent before any medication can be carried or kept at school.  Please contact your school nurse if you have any questions.
